Wrist Arthrodesis with Intramedullary Pin Fixation



Abstract
The relief of pain is the prime purpose of wrist arthrodesis in various pathologic conditions involving wrist joint.
Of the many operation that have been described for obtaining wrist fusion, most have employed a dorsal approach and included the use of a bone graft. The pseudoarthrosis and fracture are major complications of the use of any graft. The method of Millender & Nalebuff (1973), with intramedullary Steinmann pin fixation, have overcome many problems developed in the course of wrist fusion.
This study is a review and analysis of 9 cases of wrist arthrodesis using intramedullary pin fixation at Department of Orthopedic Surgery of Chung-Ang University Hospital from January. 1977 to June, 1982. 7 cases were male and 2 were female. Their age at operation ranged from 24 to 55 years. The causes of wrist fusion were post-traumatic arthritis (4 cases), tuberculosis (2 cases), rheumatoid arthritis (1 case), brachial plexus palsy (1 case), and crushing injury of the forearm (1 case).
The results of treatment were assessed by X-ray and clinical examination. The results were obtained as follows;
1) Average fixation degree of wrist arthrodesis was 14.1¡Æ dorsiflexion. Bilateral wrist arthrodesis in one patient also showed satisfactory result on functional aspect.
2) The average fusion time was 2.6 months on X-ray follow up.
3) In the 6 cases, the ulna was exposed subperiosteally and distal 2 cm was resected. It was done to avoid painful rotation and also to prevent impingement of the distal end of the ulna upon the carpus.
4) The final results were evaluated according to Straub & Ranawat(1969) formula; the overall results were graded as excellent in 6 cases(67%) and good in 3 patients(33%).
5) The main advantages of intramedullary pin fixation is a short recuperation time, so that activity, such as walking with crutches, is not lost with rigid internal fixation.
